Path 2 Life Care is a men-focused NDIS provider across Sydney and Melbourne. Accommodation is at the heart of what we do, supported by community access, personal care and pathways to work, all built around helping men live well and independently.
Finding the right home is where it starts. We specialise in supported accommodation for men, matching each person to a home and housemates that genuinely suit him, with support that flexes from a few hours to around the clock.
LONG TERM
Daily support in a shared or individual home, with our team helping each man build skills and live the way he wants. Support is available around the clock where it is needed.
ACCESSIBLE HOMES
Purpose-built, accessible homes for men with higher or complex needs. The dwelling is provided through our SDA partner and Path 2 Life Care delivers the support inside it.
SHORT STAYS
Short, supported stays that give family carers a break and give participants a change of scene, with everything taken care of.
BRIDGING
A supported place to live while waiting for a confirmed long-term home, so no one is left without the right support in between.
TAILORED
A living arrangement designed around the individual, blending the right mix of support, housemates or host arrangements to suit how he wants to live.
IN THE HOME
Active overnight and sleepover support so help is on hand through the night, with personal care, medication prompting and reassurance.
The Man Cave is our community centre built for men, a relaxed space to drop in, take part and belong. Through community access support, our team helps men get out into the things they enjoy, from social outings and group activities to appointments and learning new skills. It is community participation that feels like mates spending time together, not a program.
Personal care is delivered with dignity and respect, helping each man manage daily routines while keeping his independence. In our supported homes it is part of everyday life, provided quietly by support workers who know him well.
Work brings routine, pride and connection. We help men build the skills and confidence to step into supported or open employment, at a pace that suits them, often starting with the activities and routines built at the Man Cave.
